1. what's the best way to cut? i tried with those mixers, pieces are too small, with knife so much work... any other idea?
2. do i need to cook it? (they eat it raw, but...)
3. add garlic? if so how thinly sliced?

A hand mincer can be bought for around 30 bucks and a electrical mincer for around $180.
I am lucky i borrow the one from my work, takes around 40 minutes to make 5 kilo's of beef heart mix.
Dont cook the beef heart, keep it raw.
Just watch when adding garlic as too much they discus will almost reject it completly!
if adding a small amount of garlic to the beef heart mix, you want it to be as fine as possible.

A hand mincer is the best to cut them down.
But if you're going to feed small fry I suggest use a blender, it will make a perfect size for the fry to eat.
Keep them raw. The only thing you cook is carrot and spinach. Just boil them. This is just to make them softer and easier for the fish to eat and digest.
Add garlic , I just chucked them in thru the hand mincer or blender.
Good luck
