Flat red poop with a white stripe

Hi Guys This poor little guy is pooping something that looks chronic.
It looks like his insides. I moved him into the hosiptal tank today but he banged and bumped his way around the tank before I caught him so he could have easily have internal hemmorhaging. The reason I moved him was because he was hanging around the top of the community tank looking really dark. When I checked the water parametres I found the nitrite levels very high at just over 5mg/l. He also was flicking so I have treated the tank hospital tank with prazi. I have also added a little salt to the water.

thanks illusn, what sort of worm might it have been?
he didn't show signs of worms until yesterday when he did a clear poop - best get the big tank wormed

in all honesty I've never seen anything like that before, it looks like a flat worm so prazi would be the best start.
